Confirms your consent for your child to take part in program activities and outlines what participation involves.
Guided Outdoor Fitness & Movement — Participation & Safety Form
Confirms your child is able to take part in general physical activity and lists anything the instructor should know for safety.
Healthy Kitchen & Food Preparation — Participation & Food Safety Form
Covers food preparation participation, adult supervision at home and any foods to avoid.
Accessibility & Learning Support Information
Optional. Share information that helps your student participate and learn successfully. This is supportive educational information — it is never required and never affects enrollment.
Parent Goal & Student Support Information
Optional. Tell us what you would most like your child to work on so we can support them well. Goals are educational and skill-building — they are never required and never affect enrollment.

Youth Code of Conduct
Participants are expected to treat instructors, other students and program spaces with respect.

Student portal accounts are for the enrolled student's personal learning use only.
Educational & Wellness Notice
Enhancing Learning & Enrichment Academy, an educational program of Enhancing Body & Mind, provides educational and wellness-focused programs designed to support learning, movement, mindfulness, healthy habits, and personal development. These programs do not provide medical care, mental-health counseling, psychotherapy, physical therapy, occupational therapy, or other licensed clinical services.
